- Pence Form

- New York Sick Time Notice
You can find these forms on our Resources page, or download them altogether here. We'll have extra copies on hand when you come in for orientation/registration. Do NOT fill out the bottom of the Pence Form, or the back of the i-9 Form
Step 2. Bring in your ID(s)
The I-9 Form is for the government and here's a list of documentation they require. You must be eligible to work in the US. When you come in, you MUST bring these with you, or else we won't be able to register you and get you into our legendary database. Oh, and they must be originals, no photocopies... and they must be unexpired. It's the law - better do as they say.
SAG/AFTRA members be sure to bring your current SAG/AFTRA card.
Step 3. Attend a Central Casting Orientation/Registration
You must arrive for each registration/update time no later than the scheduled time!
With your completed forms and correct identification documents, you're ready to come visit us for orientation/registration. It takes about 1 hour, but sometimes takes longer. Please bring a pen and be PROMPT, we start on time. If you're not seated when we begin, you'll unfortunately be turned away.

Important information for Minors (background talent under 18)
Every minor performer under the age of 18 must obtain a valid one year Employment Permit for a Child Performer through the New York State Department of Labor to be eligible for employment. Information on how to obtain this permit can be found here. In order to register with Central Casting, minors are REQUIRED to have a current (i.e., UNEXPIRED) one year child performers permit physically with them at registration.
If the minor performer has never applied for a work permit previously, then a temporary 15-day work permit can be printed after completing the online application. If the minor performer has already printed a 15-day work permit, the minor must physically have an unexpired one year permit on set. The production company must keep this permit on file. If the minor is not physically in possession of a current (i.e., unexpired) work permit, then the minor is not legally allowed to work; failure to comply with this requirement could result in a fine.
IF YOU ARE NOT A U.S. CITIZEN:
If you have a sponsor, we cannot register you.
